Student Fuel Reimbursement

Please fill out this form and click submit with in 30 days of your trip. If submitted more that 30 days past your last trip date, your reimbursement will be subject to an approval process.
 
 
Please let us know which dates you are requesting fuel reimbursment for below with in 30 days of your last trip.   If reqeusted after the 30 day window it will be subject to an approval process and may be delayed. 
 
 
 
 
 
 
 
If you would like your reimbursment direclty deposited in to your checking or savings account please complete the "Direct Deposit Authorizationn" form and email to treasurer@heritageroscoe.org or put in the treasurer's mailbox in church office.
